To become a member, you MUST be a licensed Maine artist. Fill out this form and email to info@maineassociationforsafetattooing.org. WE MUST HAVE THIS FORM TO PROCESS YOU INTO OUR MEMBERS!!  This is also the mail payment address for PayPal. Please do not send payment without the completed form!!!
